Box 6 - ID Number: Provide your Florida driver's license number or identification card number. If you do not have either of these, please provide the last four digits of your Social Security number. If you do not have any of these, write the word "NONE".
Box 7 - Choice of Party: Select the political party whose primary you would like to vote in. Florida is a closed primary state. You may only vote in the primary whose party you are registered for on election day, this only applies to primary elections.
Box 8 - Race or Ethnic Group: This box is optional.

Send the completed form to your local election official. To register by mail, the form must be postmarked by the voter registration deadline. Note: Florida State University is located in Leon County.
If this is your first time registering to vote in Leon County and you are mailing the application, you are required to show proof of identification when you vote, or mail in a copy of your identification with your voter registration form. Proof of identification includes: a current photo identification, a current utility bill, bank statement, government check, paycheck, or government document that shows your name and address. How to register to vote in person:
Visit the Leon County Supervisor of Elections Office at 2990-1 Apalachee Parkway, Tallahassee, FL 32301, or any participating branch sites. Note: You may only register to vote in Leon County at the listed locations.
If it is your first time registering, bring a government-issued ID. (You do not need a government ID to register to vote, but it will expedite the process. You may still register to vote at the listed locations without a government ID.)
